This is a general outline of the questions you will find in each of the main application forms.
Note that due to dynamic branching, the exact questions you see will likely not match up 1:1 with this sample.
Pre-Qualification
- Basic Information
- Organization's Name
- Organization's Physical and Mailing Addresses
- Primary Point of Contact (Name, Email, and Phone)
- Primary Point of Contact at the FSF (Name)
- EIN and number of years in operation.
- General operating financial disclosures.
- Proposal Highlights
- What is the proposed program? Briefly describe.
- Is it a new or existing program, or an expansion of an existing program?
- How does this initiative fit into the organization's strategic growth plan?
- Who are the people living with disabilities?
- Why are they having difficulty living independently or purposefully as a result?
- How will this project provide long-term stability for its participants?
- How will participants engage in experiential learning that builds skills for independence?
- Statement of Alignment - Please fill in the template provided for this particular project.
- Mission Alignment - Evidence of how your mission fits with that of the FS Foundation.
Application Part 1
- Overview of Funding Request
- What is the total projected cost?
- Over what period of time are you planning to operate the project?
- How much total funding is needed in just the first year?
- How much are you requesting from FSF to operate it in the first year?
- How much do you expect to receive from other funders in the first year?
- How many additional years will you be needing funding?
- How much are you requesting from FSF over that additional time frame?
- How much do you expect to receive from other funders over the additional time?
- What is your plan for this project to be independent of FS Foundation funding?
- How will this project improve your organization's operations and strategic plan?
- What is your plan to use earned revenue towards offsetting project costs?
- Please submit a detailed budget for the project, including all pledged, secured, or prospective funders; a line by line description of required roles, their significance, number of hours required and cost per role; the project's total cost, and proportion requested of the FS Foundation.
- Overview of Demographics Served
- Who are the targeted beneficiaries of the proposed initiative?
- How many total participants will be involved in your project?
- Of those, how many will be living with ADA disabilities?
- Of those, how many will be living with disabilities severe enough to keep them dependent on others most of the time?
- What acute or chronic disability are they currently facing?
- What consequences of that disability are they actively facing?
- What are the current barriers to your participants that limit their ability to overcome their consequences and live independently and purposefully?
- How will this project provide long-term stability for its participants?
- How will participants engage in experiential learning to form purposeful skills?
- How will they show that this project has helped them maintain more stable lives?

- Project Context
- Briefly describe the proposed initiative.
- Is it a new or existing program or an expansion of an existing program?
- How does this initiative fit into the organization's strategic growth plan?
- How will your organization's qualifications and capacity ensure success?
- How will this project affect what currently exists to make people feel accepted?
- Please provide a list of specific goals for the project.
- Please state your realistic objectives to reach each goal.
- Please describe your tangible outcomes expected.
- Please list the measurement tools to reliably measure success by the numbers.
- Why is this project needed to increase independence and sense of purpose for all people living with mental or physical disabilities, often excluded from society?
